As an Alliance Member, if you or your spouse suffers a Heart Attack as defined, you will receive the Heart Attack Benefit of $5,000*. "Heart Attack" is defined as a myocardial infarction that has caused the death of heart muscle.

Coverage may not be available in all states. Not available in Washington State to members joining on or after 9/15/2005.

**Written proof of loss must be furnished to Alliance Member Services within 90 days after the date of such loss.

*Such payment will be made only after all the requirements outlined under the definition of Heart Attack have been met. You can use the funds in any way you wish to best meet your needs.

Insurance coverage, underwritten by The MEGA Life and Health Insurance Company, Home Office: Oklahoma City, OK, begins automatically on the date you become an Alliance Member and is subject to the limitations and exclusions listed above.
